| FACILITY NAME (DBA) | The trade name or “Doing Business As” name of the cargo tank facility. |
| APPLICATION DATE | Indicates the date of the application for the cargo tank number. |
| PHYSICAL ADDRESS | The physical location of the cargo tank facility, including the street, city, state, zip code, and phone number of the facility. |
| MAILING ADDRESS | The mailing address of the cargo tank facility, including the P.O. box or street, city, state and zip code. |
| FUNCTIONS | Describes what cargo tank related functions a facility performs. For example: External Visual Inspection, Internal Visual inspection, Lining Inspection, Pressure test, Leakage test, Thickness test, Manufacture, Assembly, Repair, or Certification. |
| VEHICLES | Indicates what vehicles the functions described above are performed on. |
| WHERE DO YOU USE TESTING/ INSPECTION EQUIPMENT? | Indicates whether the cargo tank facility does on-site or mobile testing/inspections. Could also indicate a facility does both. |
| PROCESSING AGENT | The name and address of the cargo tank facility’s processing agent. |
| RESPONSIBLE PERSON(FACILITY LOCATION) | This section contains information on the person responsible (such as an owner or manager) for the cargo facility. It includes identifying information, such as title, position, first and last name, and contact information such as phone number, fax number, and email address. |
| DESIGN CERTIFIED ENGINEERS | Indicates the names of a facility’s Design Certified Engineers and the type of certification they have. |
| NON-EMPLOYEE DESIGN CERTIFIED ENGINEERS | If a facility uses or contracts with an outside, non-employee DCE, this part of the report will show the Cargo Tank#, company name, and type of certification of the nonemployee DCE. |
| STAMP INFORMATION | STAMP TYPE: Indicates if a facility has a “U” stamp or an “R” stamp. CERTIFICATION #: Shows the certification number of the stamp. AUTHORIZATION DATE: Date the stamp is authorized. EXPIRATION DATE: Date the stamp expires. |
| CERTIFYING OFFICIAL | Indicates the Name and Title of the Certifying official, along with contact information such as phone number or email address. |
