LOCATION: The specific location in which the crash occurred. (Only first 24 characters printed) COUNTY: Indicates the County where the crash occurred. CITY: Nearest city to which the crash occurred. (Only first 11 characters printed) STATE: State in which the crash occurred. See State Abbreviations [Opens in New Window] INJURY: Total number of persons injured as a result of the crash. FATAL: Total number of fatalities as a result of the crash.. TOWAWAY: Indicates whether a vehicle involved in the crash was towed away. SEQUENCE OF EVENTS: The three digit alphabetic code describing the sequence of events of the crash. There can be up to 4 of these codes. (See Sequence of events, below.) HAZMAT: Indicates if there was any Hazmat involved/released in the crash. |

NAME: Name of the motor carrier. (First 13 characters are printed)Note: This is an abbreviated version of the carrier ID reported on the crash report. It may differ somewhat from the name and location reported in the Carrier Operations and Safety Ratings report due to reporting variation. If radically different, this may indicate a terminal address, a leased/rented vehicle, or an error. CITY: City where the Company is located. See Note under Carrier Name.
