DATA DEFINITION
Data Element Description
PHYSICAL ADDRESS Physical location and telephone number of the company's safety office.
COUNTY NAME County of the company's safety office.
MAILING ADDRESS Mailing address for the company.
EIN Employer Identification Number / Tax ID number of the company
STATUS Represents carrier's status in MCMIS. (Active or Inactive. If Inactive, the DATE INACTIVATED will be printed on report.)
CLASS Work classification. (Authorized For Hire, Exempt For Hire, Private, etc.)
CARRIER TYPE Type of Operation. (Carrier, Shipper or Both Carrier & Shipper)
CARRIER OPERATION Carrier Operation. (Interstate or Intrastate)
SHIPPER OPERATION Shipper Operation. (Interstate or Intrastate)
CARGO Description of cargo hauled by this Carrier. A maximum of three cargo types are printed.
HAZMAT C Type of Hazardous Material carried by interstate and intrastate motor carriers. Up to 3 hazardous materials may be printed. "B" indicates that the cargo is carried in Bulk. "N" indicates that the cargo is carried in Non-Bulk. "A" indicates cargo is carried both in Bulk and Non-Bulk.
HAZMAT S Type of Hazardous Material shipped by interstate and intrastate shippers. Coded same as HAZMAT C. Up to 3 hazardous materials may be printed. "B" indicates that the cargo is shipped in Bulk. "N" indicates that the cargo is shipped in Non-Bulk. "A" indicates cargo is shipped both in Bulk and Non-Bulk.
DRIVERS Total number of drivers employed by the company.
POWER UNITS Total number of power units owned, trip-leased and term-leased by the company.
TRUCK UNITS Total number of truck units owned trip-leased and term-leased by the company.
BUS UNITS Total number of bus units owned trip-leased and term-leased by the company.
MCS-150 DATE Indicates the date of the most recent MCS-150 on record for the company.
NEW ENTRANT: Indicates whether the company is currently in the New Entrant program.
NEW ENTRANT ENTRY DATE: Indicates the date the company entered the New Entrant program.
SAFETY AUDIT If there is no Safety Audit data for the company, the message NO SAFETY AUDIT FOR THIS COMPANY. Otherwise, this table contains Safety Audit information regarding the company, including:
DATE OF THE AUDIT: Date the Audit was conducted
MILEAGE: Mileage recorded at the time of the Audit
PASS/FAIL RESULT: Indicates whether the Safety Audit was Passed or Failed
SI CODE: Code that identifies the Safety Inspector.
DEFICIENT FACTORS: Factors rated Conditional or Unsatisfactory will appear in this section, along with a description, question code, and the relevant Section of the CFR. Factors are 1=General, 2=Driver, 3=Operations, 4=Maintenance, 5=Hazardous Materials, or 6=Accidents.
EXPLANATION OF SCORING METHODOLOGY: This table explains the Scoring Methodology used to determine whether the Safety Audit was passed or failed. It contains the Factors, Critical and Acute Failed Questions, Performance Test Status, Total Points, and Factor Status.
FAILED QUESTIONS (ACUTE / CRITICAL): Violations classified as critical as a result of a review are those that relate directly to the company's management and/or operational controls.Violations classified as acute as a result of a review are those so essential that noncompliance is obvious and requires immediate corrective actions by a motor carrier regardless of its overall safety posture.

CRASHES

REVIEW DATE: The date of the review during which the mileage was collected.
REVIEW MILEAGE: Total miles of truck or bus travel during the 365 days preceding the last review.
TOTAL RECORDABLE NUMBER: Total recordable crashes (i.e., crashes occurring during the 365 days preceding the last review). Crashes are designated recordable if they meet the following Federal reporting criteria: At least one fatality or one injury or a vehicle was towed as a result of the crash.
TOTAL RECORDABLE RATE: Rate of recordable crashes per million miles.
PREVENTABLE RECORDABLE NUMBER: Total preventable recordable crashes. Crashes are considered preventable if driver action or vehicle condition could have prevented the crash. This field is no longer used.
PREVENTABLE RECORDABLE RATE: Rate of preventable recordable crashes per million miles.
MILEAGE YEAR Calendar year representing the carrier-reported mileage from the MCS-150. This section will not print if no data is available.
CARRIER-REPORTED MILEAGE Total fleet mileage to nearest 10,000 miles for the last calendar year, as reported by the carrier on the MCS-150 form.
