Order Form Instructions

  1. Select File Extract & Format
    Place an "X" in the box corresponding to the File Extract(s) you wish to order.
    This order form may be used to order the following reports on CD-ROM:
  • Census File
  • Crash File
  • Inspection File
When ordering a Crash File Extract, please specify the year or years you wish included in the space provided (up to three years may be chosen). Currently, FMCSA State-reported crashes are available from years 1993 to present. If you do not indicate which year(s) should be included, the CD-ROM will contain the most recent calendar year available.
When ordering an Inspection File Extract, please specify the year you want extracted. (Inspection File Extracts are only available one year per extract.) If you do not indicate which year you want, the CD-Rom will contain the most recent calendar year available.
When ordering a Census File Extract, you do not need to specify year information.
  1. Provide Your Mailing Address
    In the space provided, tell us where to send the output. Be sure to include the name and telephone number (including area code) of a person we can contact should we have questions about your order.
  1. Calculate Cost
    1. Enter the costs for each file extract ordered.
    2. Add the costs of the extracts to determine the total cost of your order.
    3. Enter the total cost in the box labeled "Amount of Check."
    A check/money order for the total cost of the order must accompany your order form. Make the check payable to:

If you would like to order a hardcopy CSP, follow the directions below.

Order Form Instructions
  1. Specify Carrier Information
    In the space provided, enter the full name and complete address of the carrier to be profiled. The carrier’s USDOT number and/or MC/ICC number are requested, and will help us to process your order accurately.

    If you wish to order more than one CSP, provide complete information for all additional carriers on separate sheets of paper.
  1. Provide Your Mailing Address
    In the space provided, tell us where to send the CSP. Be sure to include the name and telephone number (including area code) of a person we can contact should we have questions about your order.
  1. Calculate Cost
    CSPs cost $27.50 each. See Price List. [Opens in a New Window]) You may, however, discount the total cost of your order by:
    1. 15% when ordering 2-6 CSPs
    2. 20% when ordering 7-12 CSPs
    3. 25% when ordering 13 or more CSPs
    To be eligible for the discount, all CSPs ordered must be shipped at the same time and to a single location.

    Determine the total cost of your order by following these steps:
    1. Multiply the number of CSPs ordered times $27.50.
    2. Enter the result in the "Initial Cost" box.
    3. If you are ordering:
      1. 1 CSP, enter "0" in the "Discount" box.
      2. 2-6 CSPs, multiply "Initial Cost" by 0.15; enter the result in the "Discount" box.
      3. 7-12 CSPs, multiply "Initial Cost" by 0.20; enter the result in the "Discount" box.
      4. 13 or more CSPs, multiply "Initial Cost" by 0.25; enter the result in the "Discount" box.

    Subtract "Discount" from "Initial Cost"; enter the result in the "Amount of Check" box. A check/money order for the final cost must accompany your order form. Make the Check payable to: COmputing TechnologieS, Inc.
  1. Choose Media Select the medium on which you wish to receive the CSP by placing an "X" in the appropriate box. CSPs are available on the following media:

    • Printed report.
    • E-mail (report must be 650kb or less).
    If you choose e-mail, use the space provided to tell us your e-mail address.

    1. Indicate the type of Personalized Report you wish to order. To select the report type desired, mark an "X" in the appropriate box. Mark only one box.
      1. If you select the Personalized Crash Report, specify the calendar year(s) to be covered by the report. (Up to three years may be specified.) Currently, State-reported crashes are available for the years: 1989- present. If you do not specify a year or years, the report will cover the three most recent calendar years available.
      2. If you choose the Personalized Census Report, you do not need to specify a year.
      3. If you select the Personalized Inspection Report, specify the calendar year to be covered by the report. (Only one year can be specified.) Currently, Inspection data is available dating back to 1989. If you do not specify a year, the report will cover the most recent calendar year available.


    1. Select Additional Data Elements
      The core elements always provided in Personalized Reports are listed below.

      If you wish, you may include up to four additional data elements in your report. Select additional elements for Personalized Census Reports from the Census File Elements, for Personalized Crash Reports from the Crash File Elements, and for Personalized Inspection Reports from the Inspection File Elements.

      Write the names of any additional elements you select in the spaces provided in Section A of the order form.

  1. Describe: Elements Wanted In Report
    In the space provided describe which carriers, crashes, or inspections should be included in your report.

    For example, do you want all carriers in the MCMIS Census File or only carriers in the State of Maryland? All crashes reported in 1994 or only crashes that occurred in Texas during 1994? Do you want all inspections performed in VA in 2002, or all inspections where there was a vehicle placed Out of Servie in 2006.

    You must base your description on data elements available in the proper file documentation. (Please note that the State-Reported Crash File contains data from 1989 to the present.) For example, you cannot ask for a Personalized Census Report covering all vehicles that are the color "yellow" because color data are not available in the Census File. You can, however, ask for a report covering all carriers operating one hundred or more vehicles, since fleet size is one of the data elements in the Census File.

    In describing the carriers you wish to include, you may use up to four Census File Elements.
    In describing the crashes you wish to include, you may use up to four Crash File Elements.
    In describing the Inspections you wish to include, you may use up to four Inspection File Elements.
    In the following examples, the highlighted words are data elements or their values.

    For example: "Include all carriers located in the State of Maryland," or "Include all private carriers whose current safety rating is conditional," or "Include all crashes which occurred in Texas during daylight where the driver appeared to have been drinking,"or "Include all Inspections from 2002 that were roadside inspections performed by the Midwestern Service Center.

  1. How The Data Elements Should Be Sorted
    Sorting refers to the order in which carriers/crashes/inspections will be displayed in your report. By placing an "X" in the appropriate box, you may select the standard sort format or specify your own format.

    Standard Sort Format: Elements are grouped by State and listed alphabetically by carrier name within each State.

    Other Format: If you choose to specify your own sort format, describe how the elements should be sorted in the space provided. You may use up to four data elements from the relevant file documentation in your description. The following examples illustrate how the order in which you list these elements affects the appearance of the report. The highlighted words are data elements.
If you write in Section C:
Sort the carriers by carrier classification, state, and carrier name.
